Personal Health Record PHR
A Personal Health Record or PHR is a health record where health data and medical information related to the medical history of a person is maintained by an individual. Personal Health Record (PHR) is Internet-based and allows people to access and coordinate their health history and information and make it available when is needed.
It is similar to EHR or EMR health records in their core base. But Electronic Health Record and Electronic Medical Record are widely used by institutions (such as hospitals or health centers) and contain medical data entered by healthcare providers or billing data to support insurance claims.
The main difference of PHR is that all clinical data and medical information are maintained by the patient. The mission of a PHR is to provide a complete and accurate file of an individual’s medical history which is accessible online. The health data on a PHR might include patient-reported outcome data, personal medical notes, lab results, data from electronic devices or even digitized medical records.
Electronic Medical Record EMR
An electronic medical record (EMR) refers to the systematized collection of patient and population electronically-stored health information in a digital format. It is a digital version of the traditional paper-based medical record for an individual. The EMR represents a medical record within a single facility, such as a doctor’s office or a clinic.
